D-SNP stands for Dual-Eligible Special Needs Plan (D-SNP). To be dual-eligible means that you qualify for both Medicare and Medicaid benefits.

Do I need to pick a Primary Care Provider (PCP)?

With a D-SNP plan, you may need to pick a PCP depending on whether your network is an HMO or PPO.

Do I need a referral to see a specialist?

With a D-SNP, you don't need a referral as long as the specialist is in-network. You will need a referral to see an out-of-network specialist.

What if I want to see a provider that is out-of-network?

You can choose to see a provider that is out-of-network, but you will likely pay more for your care.

Can I choose a separate Part D plan with my Wellcare D-SNP?

You cannot choose a separate Part D plan with a D-SNP, since pharmacies are part of the network.
