Review these helpful instructions to learn how to access your plan details and documents, including the Annual Notice of Change, Evidence of Coverage, Summary of Benefits, and Drug List (Formulary). 

To get started, select your state from the drop-down on the Member homepage

How to Use the Find My Plan Tool

  • From the “My plan is” dropdown, select “Medicare”
  • Enter your zip code and select “Search”
  • Locate your plan from the list of plans and select “Go to my plan details." You will be taken to the Plan Overview page.

How to Locate Plan Details and Documents from the Plan Overview Page

  • Plan Documents: locate your Annual Notice of Change, Summary of Benefits, and Evidence of Coverage documents in the “Plan Specific Documents” section of the page. You can also locate the Appointment of Representative and Medical Direct Member Reimbursement forms in the "Related Materials" section of the page.
  • Extra Benefits: locate information on extra benefits - such as dental, fitness, and hearing - by selecting “Extra Benefits” from the right sidebar. From the “Extra Benefits” drop-down menu, select the extra benefits page that you’re interested in reviewing.
  • Drug List (Formulary) and Prescription Drug Claim Form: if you have a Medicare Advantage Prescription Drug plan, you can locate your plan’s Drug List (Formulary) and Prescription Drug Claim Form by selecting “Pharmacy” from the right sidebar. From the “Pharmacy” drop-down menu, select the “Drug List (Formulary) and Other Documents” page.

  • Disaster Coverage – learn how to access coverage in the event of a disaster or local emergency.
  • Real Time Benefit Tool –  to access the Real Time Benefit tool, login to the secure member portal. If you don’t have an account, select “Create New Account” and follow the prompts.
